Hawaii

Hawaii separates the contractor classification from the well permit. The Commission on Water Resource Management publishes the Hawaii Well Construction and Pump Installation Standards. The commission's page says the State Water Code and Department of Land and Natural Resources rules require minimum standards for construction, modification, repair, and sealing of wells so that groundwater is not polluted or wasted. The well construction and pump installation permit form asks the well driller to enter a C-57 license number. The pump installer block asks for a C-57, C-57a, or A license number. Those classifications are described by the Department of Commerce and Consumer Affairs in its contractor classification list.

DCCA's classification description says a C-57 well contractor may bore, drill, excavate, case, cement, clean, and repair water wells, and may install injection wells, water well pumps and pump controls, a concrete pump base, and the waterline to an adjacent storage tank. A C-57a pumps installation contractor installs pumps and related equipment, including controls, to wells. A C-57b injection well contractor installs injection wells. The commission's permit form uses the C-57 number for the driller and allows C-57, C-57a, or A for the pump installer. The form is filed with the commission, not with DCCA.

The commission's forms page includes the well construction and pump installation permit, a process diagram, a well abandonment report, and a driller's log. The permit form's instructions say a well construction permit is issued to the licensed well driller and that a well completion report is returned after construction. The standards page says the standards were placed in the Hawaii Administrative Rules in January 1997 and revised in February 2004, with the goal in section 13-168-14 of safe maintenance, prevention of waste, and prevention of aquifer contamination. Readers should use the current form on the commission site.

A property owner should ask for the C-57 license that matches well construction and should ask whether a separate pump-installation license will be used. The permit application is a commission form, so the contractor should be the one the form expects to sign as the driller. Abandonment is covered by the commission's abandonment report, and the forms page notes that the permit requirement also applies to sealing abandoned wells. This record does not list contractors.
